Registration & Payment

You must complete a registration form in addition to submitting payment. Register online or send us
a completed paper form. Each participant must register separately but payment can be made in one

Registration Fees

Now through September 12:     $20 fee, includes event shirt

Sept 12 through Sept 19:          $25 fee, includes event shirt

Sept 20 through Oct 4:              $25 fee, includes event shirt,
                                                    only while supplies last 
Online Payment Fee: $2


Registering Offline

2014 Registration - Hard Copy Form (to fill out and send a paper copy).  Completed forms may
be scanned and sent to, or delivered to F.AVE's PTO as listed below
Derby registration forms and payments can be sent to the PTO in the following ways:

    (a)    they can be sent to F.AVE via kid-mail (red folders);
    (b)    they can be dropped off at F.AVE's front desk for delivery to the PTO's mailbox; or
    (c)    they can be mailed to the F.AVE PTO at 101 Fifth Avenue, Decatur GA 30030.
Online payment can be submitted via the PayPal link included below for online registration.
Payments by checks and money orders should be made payable to "4/5 Academy PTO."
